To Receive Your University of Houston Cougar 1Card ID Numbers for Library Access

If you are a UH student and do not have a Cougar 1Card, you will need to
obtain data that the card contains to access library resources such as
borrowing books, requesting articles and accessing electronic databases. However, if you do plan to come to campus for any reason, you are encouraged to complete this transaction in person. The Cougar 1Card Office is located in the UH University Center, Room 279. Phone 713-743-2273 for more information.

Procedure

1. The Cougar 1Card application form is located on the web at
http://www.uh.edu/cougar1card. If you do not have access to this
website, please request that a form be faxed or mailed to you by calling
(713) 743-2273.

2. Once you have printed and completed the form, please fax it back with
a photocopy of some form of picture ID to the Cougar 1Card Office at
(713) 743-2322.

3. The Cougar 1Card Office will then provide you with your Cougar 1Card
ID numbers for library services.
