Please complete the following form to reserve the following technology resources with the UHS Sugar Land Information Technology Department:

  • Multimedia Lab
  • Instructional Computer Lab
  • Mobile Computer Lab #1
  • Mobile Computer Lab #2
  • Laptop Computer for classroom instructor
  • LCD Projector for classroom instructor
  • SMART Boards for classroom instructor
  • Star Panel for classroom instructor
  • Portable Document Camera for classroom instructor
  • ZIP drives for classroom instructor
  • Polycom Sound Station
  • Sony Audio Player

TV/VCR/DVD units are found in each classroom. Questions about the TV/VCR/DVD units can be sent to the front office staff. Powered classrooms and Multimedia classrooms are reserved with the Program Coordinator, Chamal Reyes.

Once this form has been submitted and processed you will receive a confirmation by e-mail from the Technology Scheduler on the status of your request. Please allow 24 hours (not including weekends or holidays) for your request to be processed.

If you do NOT receive a confirmation e-mail within 24 hours of your request, please e-mail or call the Technology Scheduler at 281.275.3327 immediately.

The form cannot be processed unless all the fields are completed.

TV/VCR/DVD units are in each classroom. If you have questions on the units, please contact the front office staff by e-mail.

Powered classrooms and Multimedia classrooms are reserved through the Program Coordinator, Chamal Reyes. Please contact him by e-mail if you need to reserve one.

If reserving Mobile Computer Lab, how many student laptops are needed?
(Note: If you need additional technology (i.e., laptop, LCD Projector, etc., they must be reserved separately or indicated in the "comments" section below.)

If reserving Mobile Computer Lab, please indicate which peripherals you want to use:

Only TWO peripherals at a time can be used with the laptop computer. If multiple peripherals are needed, please talk with the technical staff to find out how the peripherals will be connected. The floppy drive is the default drive installed, unless you request another peripheral.
